A quick note up front: NobleMind counsels adults, adolescents, and children, and offers both in-person sessions at our Ohio office and secure HIPAA-compliant telehealth video visits. You'll choose your preferred format during intake.
Who do you see — adults, adolescents, or children?
All three. NobleMind provides counseling for adults, adolescents (roughly ages 12–17), and children (roughly ages 6–11). For minors, we partner closely with parents or legal guardians on consent, goals, and progress, while still protecting the child's or teen's therapeutic space. Children, adolescents, and adults can all be seen in person at our Ohio office or by secure HIPAA-compliant telehealth video, as long as the client is physically located in Ohio at the time of the visit.
Do you take my insurance?
NobleMind is a Medicaid-based practice. We are in-network with Ohio Medicaid and Medicaid managed-care plans, including CareSource, Buckeye Health Plan, Molina Healthcare of Ohio, AmeriHealth Caritas Ohio, UnitedHealthcare Community Plan, and Anthem Blue Cross Blue Shield Medicaid. Covered outpatient mental health visits — in person at our Toledo office or by telehealth — typically have $0 out-of-pocket cost for eligible Medicaid members. We'll verify your active Medicaid coverage during intake. If you have a different (non-Medicaid) plan, we can provide a monthly superbill for out-of-network reimbursement.
Are sessions in person, telehealth, or video?
Both. You can meet your clinician in person at our Ohio office or by secure HIPAA-compliant video visit. You'll choose your preferred format during intake, and you can switch between in-person and telehealth from one session to the next as long as you remain physically located in Ohio at the time of the visit.
How does telehealth work at NobleMind?
Telehealth visits run on a HIPAA-compliant video platform. You'll receive a unique, encrypted meeting link in your secure client portal before each session — there is no app to install. We ask that you join from a private space in Ohio with a stable internet connection. We are not able to provide telehealth to clients located outside Ohio at the time of the visit due to state licensure requirements.
Where is your office?
Our office is located at 5902 Southwyck Blvd, Toledo, OH 43614. You can reach us by phone at +1 (419) 309-6961. Detailed parking and arrival instructions are also provided in your client portal once your first in-person appointment is scheduled. If you prefer telehealth, you don't need to visit the office — your video link is delivered through the portal.
How long is a session?
Standard individual sessions are 50 minutes — same length whether you attend in person at our Ohio office or by telehealth video. Initial intakes are 60 minutes. Extended 75-minute sessions are available when clinically indicated.
What does the first session look like?
A relaxed conversation about what brought you in, your goals, your history, and how we work. If you chose in-person, you'll meet at our Ohio office. If you chose telehealth, you'll join a private video link from a quiet space in Ohio. Either way, we'll answer any questions you have.
Is what I share confidential?
Yes, with the limited legal exceptions every Ohio-licensed clinician must observe (imminent danger to self or others, suspected abuse of a minor or vulnerable adult, court order). For telehealth visits, we additionally ask that you join from a private space; we review these in detail before your first session.
How do I cancel or reschedule my appointment?
Cancel or reschedule through the secure client portal or by calling the office. We require at least 24 business hours' notice (Monday–Friday, excluding holidays and weekends) to avoid the full late-cancellation fee. The cancellation policy is the same for in-person and telehealth appointments.
How do I reschedule in the secure client portal? (step by step)
Use the portal to pick a new appointment time and your preferred format (in-person or telehealth). Every step happens inside the secure portal:
- Sign in to the secure client portal using the email and password you set during intake.
- From the left sidebar, open “Appointments”.
- Find the upcoming session you want to change and click “Reschedule” next to its date.
- Choose whether you'd like the new session to be in-person at our Ohio office or by telehealth video.
- Pick a date and start time from the available openings, then click “Continue”.
- Review the new time and format, then click “Confirm reschedule”.
- Wait for the green “Reschedule confirmed” banner; you'll also receive an email/SMS confirmation per your contact preference. Telehealth bookings include the secure video link in the portal.
- If no suitable time is available, click “Request a different time” inside the portal or call the office.
Medicaid telehealth — risks, limits, and consent
Before we bill an Ohio Medicaid plan for a telehealth (video) visit, Ohio Medicaid and ODM rules require us to review a few specific risks and get your verbal or written consent on the record. By scheduling a Medicaid-covered telehealth session you acknowledge each of the points below; your clinician will also confirm them at the start of your first telehealth visit and document your consent in your chart.
- You must be physically located in Ohio at the time of the visit. If you travel out of state, your Ohio Medicaid plan will not cover that session and we will need to reschedule.
- Telehealth has technology limits — video, audio, or internet may drop, and rare technical failures can interrupt or end a session. If we disconnect, your clinician will call the phone number on file to finish the visit or reschedule; if a meaningful portion of the session cannot be completed, no Medicaid claim will be filed for that visit.
- Telehealth is not appropriate for emergencies or active crises. NobleMind does not provide crisis, after-hours, or on-call care. If you are in immediate danger, call 911 or 988 (Suicide & Crisis Lifeline) — do not wait for your telehealth appointment.
- Privacy on your end matters: please join from a private space where others cannot overhear. We use a HIPAA-compliant, encrypted video platform under a Business Associate Agreement, but we cannot control your physical environment or any device you choose to use.
- Medicaid billing transparency: covered telehealth visits are billed to your Ohio Medicaid (or managed-care) plan using Place of Service 10 (patient at home) or 02, with Modifier 95 to indicate real-time video. Eligible members typically owe $0. We will not file a Medicaid claim for a session that didn't occur.
- You may withdraw consent to telehealth at any time and switch to in-person at our Toledo office (5902 Southwyck Blvd, Toledo, OH 43614) without affecting your Medicaid coverage or your standing as a client.
- Consent wording on file: "I confirm I am physically located in Ohio for this visit, I understand the technology and privacy limits of telehealth described above, I understand telehealth is not for emergencies, and I authorize NobleMind to bill my Ohio Medicaid (or Medicaid managed-care) plan for this telehealth session."
Do you prescribe medication?
No — we are a counseling practice. When medication may be helpful, we coordinate with your primary care provider or a psychiatrist.
Is this an emergency service?
No. NobleMind does not provide crisis or emergency care, and we do not offer after-hours or on-call support — telehealth or otherwise. If you are in danger, call 911 or 988.
