NobleMind is committed to protecting Protected Health Information (PHI). We are required by law to maintain the privacy of PHI, give you this Notice of our legal duties and privacy practices, and follow the terms of the Notice currently in effect.
Uses and disclosures we may make
- Treatment. To provide, coordinate, or manage your care.
- Payment. To bill and obtain payment for services.
- Health care operations. Quality assessment, training, and licensure activities.
Uses and disclosures requiring your authorization
For most uses other than treatment, payment, and operations, we will obtain your written authorization, which you may revoke at any time in writing.
Required disclosures
We are required by law to disclose PHI in limited circumstances including suspected abuse of a minor or vulnerable adult, imminent danger to self or others, court order, and reporting obligations to public health authorities.
Your rights
You have the right to inspect and copy your PHI, request amendments, request an accounting of disclosures, request restrictions, request confidential communications, and receive a paper copy of this Notice. You may submit any of these requests through our privacy request portal (choose Access PHI or Amend PHI). Sessions
NobleMind sessions are conducted either in person at our Ohio office or via a secure, HIPAA-compliant telehealth video platform delivered under a Business Associate Agreement. For telehealth, you'll receive a unique encrypted meeting link inside the client portal before each visit. We ask that you join telehealth visits from a private location in Ohio with a stable internet connection. Telehealth carries additional risks — such as technology interruptions and limited ability to respond to in-room emergencies — which we review with you before your first telehealth session.
Complaints
You may file a complaint with us at admin@nobleminds360.com or with the U.S. Department of Health and Human Services. We will not retaliate against you for filing a complaint.
